I have severe Crohn’s disease and have been hospitalized 14 times since starting my sentence. On April 28 I was in the SHU for investigation (I was never given a shot for anything) and went into a full body spasm. My body locked up as I was using the bathroom and I fell off the toilet. When the CO came to the door and ordered me to cuff up, I couldn’t. He said I refused to cuff up; therefore, they brought out the electric shield and when they opened the door a CO did a full dive on top of me, causing my head to hit the ground and knock out my teeth. At the time. I had four gold teeth. They laughed and joked about it and refused to give me the proper paperwork to file a claim. (The incident is documented because I had to be taken to the hospital and stayed four nights.)
I was shipped to another facility on July 1.
